I hereby authorize CA Kaplan LLC, 1300 Floyd Ave., Rome, NY 13440, hereinafter called COMPANY, to initiate debit entries at the depository financial institution named above and to debit such.
I understand that this authorization will remain in full force and in effect until CA Kaplan, LLC has received written notification from me of its termination within reasonable time (30 days), and in such manner to allow CA Kaplan, LLC and BANK to act on it.
I understand that the BANK is not responsible for any errors made by CA Kaplan, LLC and the BANK can only initiate debit and credit entries based upon information given to it by the CA Kaplan, LLC. I agree that the BANK has no liability to me if it is unable to make any transfers due to an act of God, mechanical failures, any interruptions in communication not within its control, or if sufficient funds are not given to it by CA Kaplan, LLC in the amount of the required transfer.
